Transportation Voucher Application

Our Transportation Voucher Program is designed to ease the burden of getting to and from essential medical appointments while in Los Angeles & San Diego. We understand how challenging it can be to navigate a new city during treatment, and this support helps ensure patients and their caregivers can attend appointments safely and on time.

Transportation is provided through Uber vouchers, which are sent directly to your Uber account approximately 24 hours before your scheduled appointment. To receive and use a voucher, you must have an active and working Uber account.

These vouchers are strictly intended for medical-related travel only, including trips to and from hospitals, clinics, and pharmacies. Each approved application will receive one voucher, and individuals may request up to three vouchers per week, based on appointment needs and availability of resources.

To ensure fairness and continued support for all families, vouchers must be used responsibly. Any misuse—including use for non-medical purposes or sharing vouchers—may result in immediate suspension and possible permanent ineligibility for current and future services.
